Steven Buslovich, MD, MSHCPM, is a practicing geriatrician with TeamHealth and medical director of several post-acute and long term care facilities in Western NY. He completed medical school at Stony Brook School of Medicine; completed Internal Medicine residency training at Yale New Haven Health System and Geriatrics Fellowship at Mount Sinai Hospital. He is currently serving as an executive committee member of the American Geriatrics Society (AGS). He is the recipient of several innovations awards and has published research in multiple academic journals. He is also an Assistant Clinical Professor of Medicine at the University of Buffalo School of Medicine and is actively involved in teaching and clinical research on improving models of care for frail populations. As a subject matter expert on frailty he recently participated in a United Nations Consultation on digital innovation, health and rehabilitation at Windsor Castle, UK. His passion is to develop new innovations for the aging population and their respective caregivers. He is the co-Founder of Patient Pattern, a health IT company that developed frailty risk assessment and decision support software for health professionals and risk-bearing entities caring for individuals with chronic and advanced illness. Patient Pattern won the AMDA Shark Tank Innovation Award at the 2018 annual conference.
