This session will demonstrate ‘how to’ incorporate AMDA clinical practice guidelines, pocket guides and teaching slides into practice in the PALTC setting. Using an interactive approach with small group discussion, participants will use a systems analysis approach to recognize, assess, treat and monitor strategies for implementation of evidence-based care within the PALTC setting. Understanding the facility system when implementing practice change can promote pragmatic and quality patient centered care for increasingly complex and frail residents from all members of the health care team.
Review available resources (AMDA's clinical practice guidelines, pocket guides, and teaching slides for the treatment and management of pain, heart failure and pressure ulcers and other wounds) in the PALTC setting.
Describe appropriate strategies for the management of chronic pain (including opioid analgesia) in the PALTC setting.
Discuss the importance of incorporating frailty inhale interdisciplinary management of chronic pain, heart failure and pressure ulcers and other wounds in the PALTC setting using validated assessment tools.
Discuss strategies with which to incorporate clinical products in everyday practice for all members of the interdisciplinary care team.
