Description

This case-based, interactive session will provide updates on deprescription interventions and review the role of electronic medical records, pharmacist collaboration, quality measures, and policy in relation to optimizing medications in PALTC setting. Presenters will discuss practical approaches for engaging patients, caregivers, and interdisciplinary teams, illustrate the use of tools currently available, and provide strategies to overcome existing barriers in PALTC deprescription.

  • Discuss updates on current evidence from deprescribing interventions in PALTC.
  • Discuss practical approaches for engaging patients, caregivers, and interdisciplinary teams in deprescription in the PALTC setting.
