Description

Advance Care Planning (ACP) visits in the LTC setting require skills that encompass state=of-the-art approaches, clinical experience, and helpful tools. As one of the available resources, the AMDA PALTC ACP Tool Kit includes an ACP discussion guide as well as a template for documentation of an ACP visit. The purpose of these tools is to assist practitioners in performing and documenting these complex discussions with patients and families. This session will include introductory didactic material on ACP visits and the use of the AMDA ACP Tool Kit, breakout group discussions of several cases, and an interactive discussion of lessons learned.

Learning Objectives: 

  • Review the relevant medical literature, helpful tools, and ethical and cultural considerations that are relevant to performing ACP visits in PALTC.
  • Utilize the AMDA PALTC Tool Kit as a resource for ACP visits in PALTC.
  • Describe when and how to carry out an ACP visit, assess patient capacity, and elicit and establish the patient's preferences and goals of care.
  • Explain effective communication techniques for ACP in PALTC.
