Description

This session will address the landscape of interoperability in PALTC and the importance of having integrated health care ecosystem. Speakers will discuss recent advances in health information exchange (HIE) in PALTC, review acronyms used in this setting, and discuss the role of HIE in creating an eventual longitudinal record where the patient is at the center of their data and take it wherever they go.

Learning Objectives: 

  • Review current landscape of electronic health records in PALTC and their status as regards to data sharing externally.
  • Describe current barriers to interoperability and elements of successful data exchange platforms during transitions of care in healthcare community.
  • Discuss the implications of 21st century cures act on data-blocking and ONC's recent effort with TEFCA.
  • Outline the CMS Data Element Library (DEL) as the first step towards building a integrated PALTC ecosystem.
