Description

This symposium will present an overview of the unique characteristics of assisted living (AL), provide practical strategies for optimal transitions, discuss elements unique to this setting regarding medication reconciliation, review the importance of advance care planning, and describe utilizing skilled nursing facility models of care in AL, with a focus on real-world practical examples and strategies. It will conclude with a panel discussion on issues faced in AL transitions and strategies to move forward.

Learning Objectives: 

  • Describe the unique and heterogenous characteristics of assisted living communities that are important to consider when designing strategies to mitigate transitions of care.
  • Utilize published tools to improve transitions of care through improved triage procedures for an acute change in condition and communication strategies for nursing staff during hand-offs.
  • Recognize the complex nature of polypharmacy and medication oversight unique to assisted living.
  • Apply some skilled nursing facility models of care to assisted living with a focus on similarities and differences between AL and skilled nursing facilities.
