ABPLM Designation Statement: This Internet Enduring Material has been pre-approved by the American Board of Post-Acute and Long-Term Care Medicine (ABPLM) for a total of 1.5 management hours toward certification as a Certified Medical Director (CMD) in post-acute and long-term care medicine. The CMD program is administered by the ABPLM.  Each physician should claim only those hours of credit actually spent on the activity.

Session Description:
As the Affordable Care Act is implemented, providers are using new strategies to achieve savings and improve care by changing how post-acute care is delivered and paid for.  New Pioneer Accountable Care Organization (ACO) data in Massachusetts will be shared along with successes and challenges. New skilled nursing facility (SNF) bundled payment programs will be described in dual eligible demonstrations projects as well as how SNFs can be proactive in their outreach efforts to risk bearing providers and health plans.  A novel shared savings plan will also be described between a special needs plan serving dual eligible and its SNF partners.

  • Describe the changing landscape for skilled nursing facilities in post-acute care.
  • Review data in the first year of an Accountable Care Organization performance.
  • Discuss criteria that Accountable Care Organizations and health plans are using to define their preferred networks.
  • Provide some examples of successful collaboration with post-acute providers.
  • Describe how skilled nursing facilities can engage community support and recognize the critical nature of these connections.
