ABPLM Designation Statement: This Internet Enduring Material has been pre-approved by the American Board of Post-Acute and Long-Term Care Medicine (ABPLM) for a total of 1.5 management hours toward certification as a Certified Medical Director (CMD) in post-acute and long-term care medicine. The CMD program is administered by the ABPLM.  Each physician should claim only those hours of credit actually spent on the activity.

Session Description:
This session will consist of a moderator and panelists representing a skilled care provider, managed care organization (MCO) payer, physician group taking risk for use of high acuity services and, a provider of a clinical program, and services within a MCO. Each will spend 10 minutes describing the value drivers from their different perspectives. Barriers and solutions for collaboration will be discussed. Case-based role play, amongst the panelists, will provide examples of how a clinician leader can engage with the payer on behalf of patients and their facility or organization.

Learning Objective(s):

  • Describe the value-based drivers viewed positively by the managed care organization (MCO) payers
  • Develop a value proposition and plan that includes these metrics to highlight their value in the marketplace
  • Discuss the programs and services they provide
  • Advocate on behalf of patients, prior to, and during admission
