Description

The overall goal of this activity is to increase participants' knowledge and competency on how to effectively collaborate with Hospice staff and colleagues by providing the most recent and up-to-date information available. Upon conclusion of this activity, participants should be able to:

  • Define the ability of nursing home residents to access end-of-life care from a Medicare-certified Hospice
  • Describe the role of the nursing home when a resident elects the Medicare Hospice benefit
  • Describe the role of Hospice in care planning and care provision to the Hospice-enrolled nursing home resident
  • Delineate the roles of the Medical Directors (nursing home and hospice) in ensuring the adequacy of end-of-life care for the Hospice-enrolled nursing home resident
