Session Description: This session will address the opportunities and challenges with successful participation in the Bundled Payment Care Initiative primarily, but not solely, from the perspective of post-acute care in a region of the country previously identified as over-utilizing post-acute care services. This discussion will speak to the selection process and the criteria used in choosing the most appropriate post-acute care providers, how the clinical strengths of the post-acute care providers were assessed and how hospital resources were utilized to help strengthen the clinical skill sets of the post-acute care providers. The discussion will also cover the implementation of the clinical strategies and business models developed for bundled payment including the use of clinical guidelines across all levels of care, use of dedicated case manager, identification of dedicated units and clinical teams within participating facilities, importance of scripting and patient education and re-hospitalization prevention initiatives. Lastly, the impact on the acute and post-acute care providers will be discussed.
