There are major opportunities in all care settings to improve the transitions of care for better patient outcome and reduced hospitalizations. Penalties legislated in the Affordable Care Act take effect in fiscal 2013 as part of the CMS Readmission Reduction Program. There were overlooked opportunities to improve compliance and reduce medication errors with each patient encounter. Long-term care providers will have a role to play in improving care and limiting avoidable hospitalizations/readmissions.
