Health care systems are transforming around the country with focus on attaining more value for the patients. Inherent in this transformation culture is an emphasis on innovation and implementation of new approaches to care both at systems and product-level. No setting is more deserving of innovation in health care delivery than the PA/LTC setting. The Society believes that Innovation and Implementation can help Society emerge as a leader in best practices in post-acute care and has assigned a workgroup to help Society emerge as a leader in innovation and implementation. A consensus among Society leaders and members is needed regarding the definition and role of innovation and implementation in the PA/LTC setting and also to set the direction for innovative initiatives for the Society. In this session, presenters will provide an overview of several case studies including unique programs for certified nursing assistant training and urgent care delivery along with examples of product-level innovation in the PA/LTC setting. Finally, presenters will engage the audience in an innovative forum to set the “innovations agenda” for the Society.
