Description

This session will present and discuss the compelling reasons why, in order to remain a viable residential choice for seniors, assisted living must evolve into a model of integrated care. Presenters from different health care and residential disciplines will articulate how integrating their respective skillsets will help to achieve improved overall assisted living performance via stabilized resident health attained only via an integrated care model.

Learning Objectives:

  • Discuss why onsite care should be the gold standard for health care in assisted living.
  • Define the role of health care professionals in an integrated care model in assisted living.
  • Describe how assisted living industry providers are preparing for geriatric population health management. List at least three quality metrics that assisted living providers should obtain.
