Description

This case-based session will review the common problem of polypharmacy and the special challenges of managing medically complex patients with complex medication lists, including numerous potentially inappropriate medications, in the post-acute setting. Difficult decisions regarding deprescribing- stopping medications that are no longer needed or harmful- are best made utilizing the interdisciplinary team in place in this setting including the consultant pharmacist. We will discuss a rational approach to deprescribing.

  • Describe the role of the consultant pharmacist in the post-acute setting.
