This session will describe the use of the Serious Illness Communication Guide - a structured tool to facilitate efficient and effective advance care planning (ACP) for patients with serious or life-limiting illness. Evidence to support its use as a means to decrease clinician and patient anxiety and discomfort with ACP or end-of-life discussions will be presented.
Participants will then be given the opportunity to conduct role play ACP discussions that are observed and mentored by faculty experienced in the use of this tool.
Establish an effective environment for shared-decision making regarding ACP.
Use a structured seven question tool to conduct goals of care discussions and establish advance care plans.
Conduct a comprehensive ACP session in 15 - 20 minutes using a structured tool.
