The session will present evidence on interventions that promote team collaboration in LTC using fall prevention and transitional care planning as examples. The presentations will include practical tools and strategies that can be adapted for other practice settings and clinical issues.

Learning Objectives:

  • Use a complexity science model to describe how staff connections and information flow drive care planning.
  • Explain local interaction strategies to staff that promote uptake of an evidence-based falls prevention program.
  • Describe tools, training, and technical assistance that promote effective transitional care delivery with post-acute care patients and caregivers.
