Description

This session will outline how to use data on infections to improve quality in PA/LTC and prevent readmissions to the hospital. Regulations and payment models vary among PA/LTC facilities. Antimicrobial therapy cost will often determine where in the spectrum of PA/LTC a patient is admitted. Antibiotic Stewardship programs are necessary in every facility and should be incorporated into Quality Assurance and Process Improvement (QAPI) Program.

We will review the management of common infections and discuss the role of key members of the intra-disciplinary team (IDT) in infection control, data sharing and action planning. Presenters will give a brief presentation followed by panel discussion with questions from the audience.

Learning Objectives:

  • Describe the types of infections managed across the spectrum of PA-LTC: LTACH, ARF, SNF, PACE and home health.
  • Identify the key components of antibiotic stewardship program across the spectrum of post-acute care.
  • Review the impact of infections on Skilled Nursing Facility Readmission Measures.
  • Discuss treatment options for infections in PA/LTC and choose appropriate antibiotic therapy when indicated for PA/LTC patients with infections.
  • Focus on strategies to decrease infection related hospital re-admission.
