Session Description: It has been predicted that “bundled payment” could eventually put 25 percent or more of skilled nursing facilities (SNFs) out of business. It has been suggested that the survivors will need to become medically advanced SNFs that deliver their portion of the bundle efficiently and with quality. This session will cover the basic clinical principles and practices, and related quality improvement approaches that make a facility viable in the era of managed care and bundled payment. Physicians need to be more proactive in coaching our colleagues who work in PA/LTC and provide them with effective approaches. This session will offer strategies and tools based on actual experience, such as comprehensive case reviews, training sessions with physicians and CRNPs, and the application of clinical reasoning and the care delivery process to improve clinical practices and become more attractive partners for accountable care organizations and other managed care scenarios in the future.
