Session Description: This session will focus on the collaborative and interdisciplinary nature of improving the quality of care and life for PA/LTC residents. It will feature presentations by the 2015 AMDA Foundation Quality Improvement (QI) award winners on topics including: POISe-Care: A Unique Model for Rehabilitation Patients, and Implementation of the Prevention of Avoidable Hospitalizations for Assisted Living Facilities: A Pilot Study as well as presentations from the winners of the 2016 Quality Improvement & Health Outcome award. These awards are centered on programs that facilities have successfully implemented to improve the quality of life for their residents. These presentations will form the basis for discussion by Paul Katz, MD, CMD, and Robert Kaplan, MD, FACP, CMD. Their discussion will serve to highlight the relationship between individual QI initiatives and larger research projects and trends that strengthen the quality of life across the PA/LTC continuum.
