ABPLM Designation Statement: This internet enduring material has been pre-approved by the American Board of Post-Acute and Long-Term Care Medicine (ABPLM) for a total of 3.5 clinical hours toward certification as a Certified Medical Director (CMD) in post-acute and long-term care medicine. The CMD program is administered by the ABPLM.  Each physician should claim only those hours of credit actually spent on the activity. 

Session Description:
Chronic conditions such as diabetes, chronic obstructive pulmonary disease (COPD), and heart failure are prevalent in the PA/LTC setting. This workshop will discuss the recently updated clinical practice guidelines (CPGs) developed to improve the PA/LTC management of these conditions. The workshop will focus on successful strategies for implementation of the CPGs, prevention of emergency room transfers and avoidable hospitalizations, and the use of selected performance measurement indicators designed for this care setting.

Learning Objective(s):

  • Explain implementation science as it relates to the implementation of clinical practice guidelines.
  • Incorporate and utilize the diabetes, COPD, and heart failure clinical practice guidelines(CPGs), and promote awareness of the graded recommendations for each condition, within their facilities.
  • Describe specific interventions that result in fewer emergency room transfers and avoidable hospitalizations related to diabetes, COPD, and heart failure.
  • Explain the use of performance measurement indicators designed for monitoring the management  of chronic conditions in the PA/LTC settings.
