ABPLM Designation Statement: This internet enduring material has been pre-approved by the American Board of Post-Acute and Long-Term Care Medicine (ABPLM) for a total of 3.5 management hours toward certification as a Certified Medical Director (CMD) in post-acute and long-term care medicine. The CMD program is administered by the ABPLM.  Each physician should claim only those hours of credit actually spent on the activity. 

Session Description:
This session highlights four unique programs designed to improve the quality of care associated with transitions that emphasize system change that is both effective and sustainable. Standardized approaches involving care coordination, personal goal setting, patient order sets, and team-based patient safety conferences and information transfer will be described. The highlighted interventions are contextualized within the ever changing environment of new payment methodologies including accountable care organizations (ACOs), bundled payment, and Medicare Advantage plans..

  • Discuss the current status of hospital to SNF transitions and the potential for better systems and outcome.
  • Enumerate current barriers to better transitional care and potential intervention to overcome these issues.
