Session Description: This session creates a forum for interdisciplinary teams (IDTs) to share their innovative programs and policies. Come hear how other teams have overcome barriers and challenges in their practice. Several programs will briefly present their best practices, and then offer expert advice to attendees and share the materials which have contributed to their programs' successes in roundtable format. This session is an excellent opportunity to network with other IDT colleagues and learn new strategies for addressing challenges in your long term care practice.
