Session Description: Emergency departments are major providers of nursing home residents. This session will describe the current state of transfers of care (in both directions) between nursing homes and emergency departments with a focus on gaps in quality and safety. It will review the latest research and ongoing research in that area. Finally, the necessary next steps to improve quality and safety during these care transitions will be discussed.
