Session Description: This session presents California's experience with the development of the CARE (Compassion and Respect at the End of Life) recommendations. These expert consensus recommendations were developed by the Coalition for Compassionate Care of California with funding from the California Health Care Foundation. They are designed to support patient centered palliative care plans as a part of the "usual care" provided for the frail patients of our nursing homes.

Discuss the tools and educational resources CARE developed to support improved palliative care.
Identify the components from the CARE Recommendations that could improve palliative care and "Dying Well" in their Home(s).
