Session Description: This session will facilitate a lively exchange among an experienced medical director, defense attorney and consultant nurse, discussing everyday strategies for your care and documentation to improve quality and reduce liability exposure. It will discuss high-risk clinical and psychosocial scenarios in the nursing home and share strategies to avoid "red flags" of charting care.
Recognize high-risk clinical and psychosocial scenarios in the nursing home (such as pressure ulcers, falls, dehydration, malnutrition, medication adverse effects, changes of condition), and optimize care and documentation to address these scenarios.
Share strategies with your team to avoid ""red flags"" of charting and care, including rote charting, failure to follow up on changes or abnormalities, notification failures, inadequate assessments, and value judgments.
Evaluate a chart with an eye to risk management, recognizing entries that an attorney would consider as risk-reducing /protective versus risk-increasing/adverse from a legal liability standpoint.
