Session Description:
Physician Orders for Life-Sustaining Treatment program (POLST) is a great advance for preparing frail patients for personalized advanced illness management. The California POLST became a standardized legally recognized tool 1/01/09. Come learn about this tool and how to implement it in your state and facility(s).

Learning Objectives(s):

  • Describe POLST
  • Discuss how POLST will improve the management of advanced illness for frail patients' across the settings of medical care
  • Examine how POLST works with other advanced illness tools like Advanced Health Care Tools, DNAR Forms, and PIT (Preferred Intensity of Care Forms)
  • List ways to address the many barriers to successful implementation of a reliable POLST program in facility(s) and community
