Session Description: This session presents three innovative programs from three different settings, all targeted at providing dignified, spiritually sensitive, and maximally comfortable end-of-life experience irrespective of the setting the individual chooses for their final care. The panel will then discuss pearls related to end-of-life care across the long term care spectrum.
Manage the transition from acute hospital to home and residential care environments for patients with delirium in a safer fashion.
Recognize factors that contribute to successful end-of-life care in residential settings, including factors that help avoid the need to hospitalize or transfer to higher levels of care.
Contrast factors related to meeting spiritual and emotional needs of dying persons to factors directed at physical comfort.
Discuss frustrating elements of your own management of end-of-life care with the panel, and share solutions to problems that will help others better manage end-of-life challenges.
