Session Description:
This session presents three innovative programs from three different settings, all targeted at providing dignified, spiritually sensitive, and maximally comfortable end-of-life experience irrespective of the setting the individual chooses for their final care. The panel will then discuss pearls related to end-of-life care across the long term care spectrum.

Learning Objectives(s):

  • Manage the transition from acute hospital to home and residential care environments for patients with delirium in a safer fashion.
  • Recognize factors that contribute to successful end-of-life care in residential settings, including factors that help avoid the need to hospitalize or transfer to higher levels of care.
  • Contrast factors related to meeting spiritual and emotional needs of dying persons to factors directed at physical comfort.
  • Discuss frustrating elements of your own management of end-of-life care with the panel, and share solutions to problems that will help others better manage end-of-life challenges.
