This session focuses on how difficulties in the transfer of patients to and from hospitals and long term care facilities represent a major dilemma in health care. This is especially true for physicians who increasingly perform either hospitalist or long term care specialist roles. The Geriatric and Palliative Care Teams from Penn State University examine the trials and tribulations of working out the wrinkles in the continuum of care by illustrating cases and offering real world solutions to identified challenges. The importance of physician leadership in directing this process and information about interfacing with hospitalists will be emphasized. Participants are encouraged to bring difficult cases for review with the facilitators and speakers.
Define 'overlap' physicians and identify one or more in your region to champion the transition process
Describe the regulations and reimbursement governing the use of Palliative Care, Hospice, LTAC, and Rehab care and be able to delineate proper placement in case presentations
Identify the barriers to obtaining various levels of care and how to overcome them
Describe the potential reasons for 'failed' placements and futile admissions from the hospitalist perspective
